An independent review into the tragic death of two-week-old Brendon Staddon, known as Child C, has called for stronger multi-agency information sharing.
Brendon’s father, Daniel Gunter, was convicted of murder in July 2025, while his mother, Sophie Staddon, was cleared of all charges.
The Somerset Safeguarding Children Partnership (SSCP) said the review found the horrific incident in a special care baby unit in Yeovil could not have been predicted but highlighted clear lessons for child safeguarding.
Key recommendations focus on stronger multi-agency information sharing, improved risk assessments, and better consideration of domestic abuse and wider family involvement.
Some strengths were noted, including pre-birth planning and early care proceedings. Immediate actions after a Rapid Review include a new information-sharing tool, updated staff training, and revised multi-agency safeguarding meetings.
A spokesperson for the SSCP said: ‘The murder of Brendon Staddon was a hugely distressing incident, and our thoughts and deepest sympathies remain with Brendon’s family and all those who were impacted by his tragic death.
‘It’s important we make every effort to learn from the events that led up to Brendon’s death. This is a comprehensive report which highlights areas for improvement as well as good practice. We have already actioned many of the recommended changes and will ensure this learning is shared widely.’
